Joe Baugher's website is pretty thorough and he has no record of the accident. I'd suggest if he doesn't have it, then there's no publically available record of it - http://www.joebaugher.com/usaf_serials/1969.html
It was a J model between approximately 1989 and 1995 before being converted to an M, it presumably happened then.
I first became aware of them about twelve years ago and to the best of my knowledge, I've never seen a combat related one on there. If it did happen in the Gulf War it would possibly explain why there's no more obvious details available on the 'net.
