This memorial can be seen at the National Memorial Arboretum, Alrewas, Staffordshire. (It is well worth a visit, but wrap up if you go in the winter)

Two fallen of Lancaster DV 168, 12th Squadron, RAF
who were killed 18th August 1943

Squadron Leader, Pilot, Frazer Burstock Slade, died aged 31. Son of Charles Percival and Elizabeth Olive Slade and husband of Salome Greta of Tavistock, Devon

Flying Officer George Ralph Carpenter, Navigator died aged 23. Son of Harold and Violet of Birmingham, and husband of Margaret Eva of Hall Green, Birmingham.
Both are commemorated on the Runnymede Memorial, Surrey
