Could be crime related, as German media say the motive was likely some sort of altercation. Perhaps this wasn't some random killing spree, but a targeted attempt on someone.
There's been a recent uptick of organised crime in East Germany, presumably connected to the meth industry of Czechia. A day before this incident, and also in Saxony, a dog walker trying to retrieve her dog from a stranger's premises was gravely hurt by an improvised mine set up by the homeowner, a drug dealer who was apparently fearing someone's revenge.

Like I've pointed out many a time, it's virtually impossible in most German states to commit even dangerous psychiatric patients against their will, and they're getting releases all the time. And much like in the case of terrorism, we're being told that this is the "price of freedom".

Yeah, I'd rather not be run over just because some big-hearted psychiatrist thought that hearing voices commanding one to kill their fellow citizens is not a sign of imminent danger.

It relies on contractual trust between the doctor and the patient, with the patient agreeing to the terms set by the doctor.

Unfortunately it is easy to abuse the trust and disregard the terms. The patient can outsmart or outplay the doctor, the doctor can fail in his evaluation of the patient, the patient can end up being unreliable in his own "self-preservation" (not taking his medication, taking them improperly, etc...).
